The practice nurse (POH-S) sees patients with a chronic physical illness, such as diabetes, astma, COPD and heart problems. She provides information on illnesses and medication, gives advice on lifestyle and does routinely examinations. The practice nurse has her own consultations hours in the practice and does home visits.
